How are the club icon and name displayed in reports?

The club icon and name are controlled through Golden Records Preferences:
- The Club Icon property determines which image appears in the report. Any image set in the template editor will be ignored and replaced by the Club Icon property setting.
- The Club Name property controls how the club name appears
- These settings affect both standard reports and the Summary Report
